Fall 2018 Lambda Phi Epsilon HQ The chapter failed to comply with national standards regarding social events.  Required HQ outcomes included social suspension through the remainder of the fall 2018 and spring 2019 semester, completion of Lambda Phi Epsilon Code of Conduct training, and attendance at Lambda Phi Epsilon's President's Orientation program. Further incidents of Code of Conduct violations will result in sanctions of increased severity. 
Fall 2018 Chi Psi IFC The chapter failed to comply with IFC standards regarding alcohol and social events. Required IFCJC outcomes included completion of educational programming on alcohol and a fine. Further incidents of alcohol  violations  will result in sanctions of increased severity. 
Fall 2018 Beta Theta Pi University; HQ The chapter failed to comply with national standards regarding social events.  Required University and HQ outcomes included social suspension through the end of the fall 2018 semester, required review of social function plans with HQ staff, submission of Executive Board Meetings and Chapter meetings to HQ on a weekly basis, completion of recruitment workshop/training provided by HQ and completion of risk management and inclusivity training provided by University staff.
Spring 2018 Alpha Chi Omega University The chapter failed to comply with University standards regarding alcohol.  Required University outcomes included the completion of an alcohol education program with 100% attendance from the chapter. 
Spring 2018 Phi Gamma Delta IFC The chapter failed to comply with IFC standards regarding alcohol and recruitment. Required IFCJC outcomes included a fine. Further incidents of alcohol and/or recruitment violations will result in sanctions of increased severity.

Spring 2018 Phi Kappa Psi University; IFC The chapter failed to comply with University and IFC standards regarding alcohol, recruitment, and new member education.   Required University and IFCJC outcomes included completion of an educational program on hazing prevention, discontinuation of all physical activities associated with IFC Bid Day and new member education,  submission of revised IFC Bid Day activities and new member education plan to Fraternity & Sorority Life staff, and a fine. Further incidents of new member education violations will result in the immediate termination of the chapter's Fraternal Organization Agreement. 
Spring 2018 Sigma Lambda Upsilon University The chapter failed to comply with University standards regarding new member education.  Required University outcomes included submission of a revised new member education plan, loss of new member presentation privileges for spring 2018, and a period of suspension from May 2-August 28, 2018. Further violations of new member education policies will result in the termination of the chapter's Fraternal Organization Agreement.  
Spring 2018 St. Elmo Hall IFC The chapter failed to comply with IFC standards regarding personal conduct. Required IFCJC outcomes included completion of an educational program on personal conduct and a fine. Further incidents of personal conduct violations will result in sanctions of increased severity.

Fall 2017 Alpha Chi Omega ISC The chapter failed to comply with ISC standards regarding recruitment. Required ISC outcomes included a fine. 
Fall 2017 Sigma Phi Society University The chapter failed to comply with University standards regarding alcohol and new member education The chapter's Fraternal Organization Agreement was terminated in Fall 2017. The fraternity was conditionally reinstated to participate in the Spring 2018 recruitment process.
Fall 2017 Pi Lambda Phi IFC The chapter failed to comply with IFC standards regarding alcohol and social events.  Required IFCJC outcomes included a fine and revision of social function safety policy.  Further incidents of alcohol and/or social event violations will result in sanctions of increased severity. 
Fall 2017 Alpha Delta Phi University The chapter failed to comply with University standards regarding programming attendance. The chapter's Fraternal Organization Agreement was suspended until May 18, 2018.
